Job Title: Information and Assistance Specialist


General Description:
Provide current information regarding opportunities and services available to older adults and individuals with disabilities within the community, assess issues and capacities of individuals, and link the available services and resources with the individuals. In addition, the case manager will provide face to face intervention services when initiated by an agency or organization for the purpose of identifying potential clients (or their care givers) and will encourage the use of existing services and benefits.

Case Coordination Unit Supervisor


Education Requirements:

Must have a BA/BS degree in social science, social work, or related field.


Job Duties and Responsibilities:

Provide on-site assistance in a variety of I & A areas such as circuit breaker, energy assistance, public aid, social security, medicare etc.

Receive inquiries and determine what service or information is required.

Determine whether services were received and met needs by following up with the client.

Arrange for escort and transportation services for those clients who require such assistance.

Maintain county wide I & A remote sites.

Develop and maintain a resource file.

Conduct home visits to the home of the senior to provide such services as the completion of circuit breakers, energy assistance applications, food stamps, Medicaid, and Medicare applications.

Assist with public aid applications.

Speak to local community groups regarding the programs of Senior Services.

Participate as member of local task forces, networking groups, etc.

Attend required trainings and attend staff meetings.

Help clients complete applications for various programs for they may qualify.

Maintain a file on each client and document all interactions.

Complete all required paperwork.

Participate in agency fundraisers.

Other duties as assigned.


Other Job Requirements:

Must have personal transportation.

Must be willing to travel to information and outreach sites throughout the county.

Must be willing to attend training sessions and meetings where an occasional overnight stay may be required.


Key Skills Required:

Detail-oriented, organized, maintains thorough and accurate records.

Experience with Medicare and Medicaid is a plus.

Good communication and writing skills. Patient listener, able to deliver uncomfortable information in such a way as to minimize adverse reactions.

Must be level-headed, able to act/interact appropriately in emotional situations.

Must maintain absolute privacy of client information/data.

Must have knowledge of computer operation and keyboard skills.

Must be able to manage own time effectively.

Ability to multi-task while keeping the proper priorities in place.

Able to maintain confidences.

Ability to establish credibility and influence with others inside and outside of the organization.

Must have a helping attitude with clients as well as peers within the organization.

Ability to solve problems effectively utilizing all of the community resources available.

Recognize the nuances of a situation and have the ability to adapt his/her personal approach to the needs of the situation.
